welcome I'm Dr Ian young with tyby Wellness out of Savannah Georgia and the American Academy of manipulative therapies Fellowship in ultrasonography today I'm here to demonstrate an ultrasound guided dry needling technique to the Patell tendon so what we're going to do is use the claras linear probe to help guide that procedure so first off we're going to get the patient lined up supine with the knee bent at at least 30° to put that Pella tendon on stretch and then we're going to line up the clarus longitudinally or in The Lax position along the length of the Patell tendon once we get that view we will look for the enthesis of the Patell tendon at the inferior pole of the patella and then we're going to take our dry needle and we're going to enter from distal to proximal so what we're going to do is take that at a very shallow angle probably less than 30° to make bony contact at the enthesis so we're going to come in again distal to proximal take the needle place it in there once we make contact with the inferior pole the patella what we're going to do then is possibly do some periostal pecking to create a greater uh healing process for a chronic tendonosis for example then we'll pull that needle out okay and that that is the ultrasound guided procedure for the Patel attendan with the help and guidance of the claras onear probe
