this short access view of the achilles tendon with the linear scanner can identify many things in the mid span of the achilles tendon here we are just proximal to its insertion and normally we would see a nice tight tendon with organized fibers but as we scan up and down through it we can see it's a significantly heterogeneous texture some bright some isoechoic and even some hypoechoic with a suggestion of some surrounding fluid these findings are really suggestive of some partial tearing of the tendon or at least ongoing tendinopathy in the area of concern in the mid-span and we can find as we scan down to its insertion to the calcaneus this achilles tendon appears fairly normal at the insertional site suggesting most of the pathology is in the midspan
