this is a case of a 12 year old male neutered domestic short-haired cat he had a history of deteriorating body condition scores so weight loss previous UTIs and where the animal had been clipped for Bloods Etc there was no hair regrowth he had a pot belly Mass was palpated mid abdomen and he had recently become diabetic here we can see the spleen at the top of the image you can see the stomach cranially and we can see some intestines caudally in the image we have this structure here which is the left limb of the pancreas it stands out a lot more than it should do it should be isoechoic with the tissue around it but we can see it's quite hypoechoic we can see the splenic vein coming down here and we can see that the tissue around the pancreas is quite hyperechoic relative to this pancreas which is enlarged with um non-smooth or an irregular margin to it and it is slightly heterogeneous in this View here we're following the pancreas a bit further and we can see that there are large abnormalities there we've got the spleen at the top and the stomach over here and we can see in some views we can see a pancreatic duct going through the center of it but we can see that it's very heterogeneous very enlarged more images of the left limb of the pancreas here where we can really see this irregular margin um the heterogenicity of it we've got the spleen in the near field View and we can see that it really stands out compared to the surrounding tissue and normally it would be very isoechoic so you would really struggle to see the pancreas but here it has a very prominent effect because it is so heterogeneous and the tissue around it is reactive and therefore hyperechoic so this is a view of the right limb of the pancreas so we have the duodenum at the top of the image and then we have the right limb below it so medial to it um again it's very heterogeneous this pancreas um and um stands out because the tissue around it is reactive but this is far more than a simple pancreatitis um this this looks very severe so still image of the duodenum at the top here in longitudinal and then below it we have this very abnormal pancreas here we have the stomach and we're following that through the pylorus up to the pylori geodenal Junction and to the duodenum and just observing that right limb of the pancreas again locally there were also some enlarged lymph nodes so the in this was on the right hand side um close to the duodenum and the pancreas not a lymph node we would normally see on an abdominal exam so therefore prominent and enlarged so there was evidence of massive changes in the pancreas pancreas was very abnormal heterogeneous with cystic structures seen throughout it was well defined but had irregular margins and was grossly enlarged and this was the mass that had been palpated in the abdomen prior to the ultrasound scan no normal parts of the pancreas were seen so the entire organ was affected a lymph node in the vicinity of the pancreas also appeared hypoechoic rounded and enlarged this could be inflammatory change or near plastic without sampling we won't get the exact diagnosis unfortunately in this case the owner decided that euthanasia would be the the kindest option
