hi I'm Brian Johnson I'm an Emergency Physician and today we're going to use the clar's pal to look at the hip you know often times people come in with acute pain to their hip and you want to see if there's a joint diffusion um you can easily uh evaluate the hip with an ultrasound and so first and foremost have the patient in Seine position they can kind of externally rotate the hip if possible in your preset you want to do the msk setting and what you do is uh in longitudinal orientation you really want to just identify the femur kind of the mid femur and that hyper aoic structure at the bottom of your image is the femur now you want to just track proximal and follow that femur up up up until you lengthen till you're hitting basically the femoral neck and then the acetabulum and that is your joint space so typically if there's an infusion you'd see hypoc coic fluid within the joint space this could be an indication for an Arthur centesis further Imaging or at least thinking about a potential septic hip or a fusion as a cause of the patients's hip pain
