MSK Ultrasound in Physical Therapy: Building Diagnostic Trust and Patient Buy-In

When most of us walk into a physical therapy clinic, our priority is simple: we want to know what is actually happening beneath the surface and how to fix it. But traditionally, rehabilitation clinicians haven’t had all the tools to provide real time feedback.

Dr. Christopher “CJ” Miller, PT, DPT, of Frontline Function, is challenging that limitation across orthopedic, primary care, and urgent care settings by placing musculoskeletal point-of-care ultrasound (MSK POCUS) at the center of his clinical evaluations. Instead of stepping in to issue corrective exercises for an injury diagnosed elsewhere, he now evaluates patients using ultrasound in addition to his physical exam.

Beyond direct patient care, Dr. Miller is dedicated to clinical education. His primary objective is to teach clinicians how to confidently integrate MSK ultrasound into day-to-day practice—bridging the gap between initial assessment, diagnostic precision, and effective rehabilitation pathways.

We recently interviewed CJ about his use of Clarius ultrasound in his practice. Watch the 3-minute video to learn more. Read on for highlights from our discussion.

Validating the Clinical Exam to Unlock Patient Adherence

According to CJ, integrating MSK ultrasound into clinical practice elevates the therapist’s role from carrying out third-party directives to evaluating soft-tissue integrity at the bedside. When direct visualization confirms physical exam findings, patient confidence shifts immediately.

The addition of ultrasound puts more weight in our ability to perform a really accurate clinical exam. If they can trust that you evaluate what’s happening really well, they’ll trust that you can get it better. ‘I’ve seen this tissue. You know what you’re talking about. Now I’m going to go ahead and fulfill this plan of care.'”

Seeing anatomy in real time replaces diagnostic ambiguity with therapeutic buy-in. When patients see the affected tissue side-by-side with healthy structures on screen, compliance moves from passive instruction to active participation in recovery.

Real-Time Validation: How Bedside Imaging Changes the Patient Experience

The clinical impact of point-of-care ultrasound is felt most clearly by the patient on the table.

For example, a year after undergoing surgery to repair a ruptured left Achilles tendon, CJ’s patient Mike Lampkin began experiencing discomfort and pain in his opposite, uninjured right ankle. Concerned about the risk of another prolonged recovery, Lampkin came in looking for clarity rather than guesswork. By performing an immediate, side-by-side ultrasound exam, Dr. Miller was able to assess tendon thickness, evaluate suspected inflammation, and compare both sides in real time.

Patient Video Interview: Mike Lampkin on Real-Time Ultrasound in Physical Therapy

As Lampkin shares in this 2-minute video, having objective imaging integrated seamlessly into a regular PT visit transformed the appointment into a transparent, collaborative evaluation that replaces anxiety with a clear, confident path forward.

It’s great to have that instant feedback and be able to have the comparison of the two tendons on the left and the right,” says Mike. It made me more confident as far as my rehab goes, and be able to have a clear path forward and not have a guessing game of what exactly is going on.”

Clarius Wireless Ultrasound: High-Definition MSK Imaging Built for Physical Therapy Practices

Clarius handheld wireless ultrasound machines deliver the resolution of conventional cart-based units in a completely untethered format engineered for daily mobility and affordability. Featuring high-frequency linear arrays optimized for musculoskeletal imaging, Clarius scanners pair wirelessly with standard iOS and Android devices, allowing clinicians to navigate smoothly between urgent care bays and rehabilitation treatment rooms. With high-definition dynamic imaging, intuitive split-screen comparisons, and intelligent, automated workflow tools, Clarius gives physical therapists reliable tissue visualization at the bedside without slowing clinical tempo.

Portability That Keeps Physical Therapists Mobile

Bulky ultrasound systems often create spatial bottlenecks in fast-moving clinics. Untethered, pocket-sized imaging eliminates those obstacles entirely explains CJ.

I think MSK ultrasound is expanding in physical therapy for a few reasons. First and foremost, I think your patients want to know what’s happening. The technology has gotten much more affordable and much smaller. So, these Clarius handheld units are wireless. If I’m going to see a patient up in our urgent care, I can have a bottle of gel in one pocket, an ultrasound probe in the other, and carrying an iPad, and I could be mobile, and I can get around a patient in a room no matter what room they’re in. I think it’s just become a lot more accessible while maintaining a high-quality image.”
