eFAST Exam with the PAL Scanner

hi my name is Tom Cook I'm an Emergency Physician practicing in Columbia South Carolina and I'm going to be demonstrating the fast exam today we have a special treat we're going to be using clar's New Pal HD3 transducer pal is an acronym for phased array and linear and this particular device has the capability of using both phased array and linear array technology and I think you'll find this very interesting so U we'll go ahead with this demonstration of the fast exam uh by starting ing uh with an examination of the right upper quadrant and of course we're looking at that potential space between the liver and the right kidney where intrarenal fluid would accumulate the next part of this examination is to go to the left upper quadrant and again we're looking for the potential space between the spleen and the left kidney to uh show the accumulation of fluid next we'll go ahead and move to the pelvis and what we'd like to do is scan down until we see the proc prostate and then angle back up uh into the perianal cavity by moving the transducer more seilat next we're going to go and get a subcostal view of the heart so what we're particularly interested is that area between the right ventricle and the liver where we would see an accumulation of fluid in the case of paric cardal diffusion now we'll go ahead and look at the patient's heart in The parisal View as well and what this particular uh preset can do is it understands what it's looking at what part of the body and we'll adjust the image and the orientation accordingly so you might notice that now this is an a cardiac preset and we're getting a really nice view of this patient's heart in that particular setting next as part of the fast exam we'll go ahead and examine both lungs looking at the plura and once again the system can understand that we're now looking at a different body area and flip from the phased array setting into the linear array setting and we can see here the rib and then the rib inter space where we see plural sliding on the patient's right and then we'll move over to the patient's left again there's a rib there's the rib interpace and we see plural sliding and of course we would be looking in these areas for evidence of num thorax so this is the extended fast exam it can be accomplished in as little as a minute to a minute and a half of time we use this uh every day in my department where we see a large number of trauma patients and you can make a very very quick assessment about a number of very important pathologies in the trauma patient and subsequently manage that patient appropriately
