so we got a young man having a uh ulnar nerve transposition today so we're going to look at his supraclavicular approach and do a corner pocket if we get a good view so there's his artery get a good look at the rib so it's kind of kind of deep right now so real easy to change the depth on the screen just pull down there we go and we just want to come in into the corner pocket here underneath and get him numbed up so i prep him up with a claw prep all right so here comes a little number shot it's going to sting on three so on three take a big deep breath one two three that a boy a little burn right here local in come in with our needle nice view of the needle coming in from lateral to medial i'm going to go down to the first rib here tell you what rocky go ahead and give me five and let's push these nerves up out of our way respiration is negative good i want to advance a little bit more get closer to the artery here give us five more rocky there you go beautiful pull back a little bit all right i'm gonna go in a little farther get closer to the right under the artery see the artery there give me five more lifting everything up 15 okay five more here usually i get more of the plexus here but since we're just doing the on the nerve transposition i'm just going into the corner pocket and just lifting the whole plexus up and just coming in and letting the local go up between the artery and the and the plexus there and then soaking this lower trunk and getting that lower trunk not worried about the radial nerve and upper trunk on this one okay rocky five more yeah i pull back a little bit okay and we'll just put a little bit right out here come right okay five right here all righty beautiful here we go so now we're finished up you can see a nice see the first rib here the artery or the lower trunk here just circled with local upper trunk up here going up higher not worried about that so much but a nice pretty view there and we're done
