Heather is here for a PRP injection she suffers from chronic neck pain related to an injury she suffered years ago and has had how many surgeries five five surgeries with insertion of hardware removal of Hardware she's been through radio frequency ablation nerve loocks trigger points as well as peripheral neurom modulation and has still not had any long-term chronic relief and we're hoping that PRP will do it for her okay so now I'm going to deposit some PRP under the cervical power spinal muscle a little pinched here okay am I in a spot that hurts you okay so I'm going into the cervical power spinal muscles to give a little bit of PRP capture image I'm just giving a little bit as I go in and out just getting trigger points done here there's my needle I'm on the lamina now and I'm moving the ne more lateral towards your articular pillar to get the region posterior of the frame in where the Mei Branch nerves are you okay Heather yeah I'm trying about to move am I in a spot that your pain comes from yes and a little bit higher and higher I'll go higher as well yeah is this a spot that hurts you as well mhm good so now I'm going to do the same thing on the other side articular pillar once again and here's a cervical frame and anterior to that pillar okay A little pinch hether am I in the spot that hurts the ne's coming lateral to the particular pillar going a little more anterior does it hurt you over here is this a spot that hurt you yeah where higher up okay a little higher okay just try it to move here okay I'm me go higher up okay right over here good so this is a trigger point probably close to C2 C3 right we're getting a lot of good stuff in here and you see it's spreading into the muscles right along the lamina and as well as lateral by the articular pillar you okay these regions yeah right around there y okay probably close to C7 okay okay Heather little pinch okay okay right here okay heard I'm sorry there's my needle that's good that's a good spot going down underneath my shoulder blades I get the pain yep it's a referred pain so I'm getting some here capture image a little more over here how's that that's good you can feel that radi okay if you want me to go left right up down just tell me no no no that's good I feel it where it rates more jelly do the other side now okay right around the same exact level right okay pinch good right over here you have trigger points I can feel it capture image decrease depth okay I'm just getting right outside of that transverse process going to go a little anterior to it to get is that a good spot or no no that's good I feel so what we did for Heather was basically some deep trigger points I didn't get exactly into the facet joints or directly onto the ep the nerves as they leave the epidural space however some of the medication of the of the PRP I should say will flow over towards the medial branch nerves as well as those exiting nerve roots and possibly reduce any inflammation but she has as you could see a lot of tender spots that she wanted me to Target and hopefully it will help her
