How to Use Clarius Auto IMT for Carotid Artery Ultrasound

Shelley Guenther, CRGS, CRCS

Carotid Intima-Media Thickness (IMT) is a key biomarker for subclinical atherosclerosis, vascular age, and overall cardiovascular risk assessment. In this step-by-step tutorial, learn how Clarius Auto IMT uses automated AI tracing to provide fast, reliable, and reproducible carotid wall measurements right from your handheld scanner.

Compatible Scanners: Clarius L7, L5, and PAL scanners (using the Carotid Preset).

Step-by-Step Workflow:

Positioning: Locate the carotid artery and center the beam approximately 1 cm proximal to the carotid bulb/flow divider.

Alignment & Zoom: Ensure the transducer is perpendicular to the posterior arterial wall, then zoom in to clearly resolve the intima and media layers.

Automated Measurement: Open the Tools menu and select IMT. The AI automatically freezes the frame, places a yellow Region of Interest (ROI) box, and generates a precise trace with average thickness displayed in the upper left.

Customization: Resize or adjust the ROI box to match your specific clinic screening protocol.

Documentation: Save images and calculated values directly to patient reports and the Clarius Cloud.

Key Benefits:

Eliminates manual tracing errors with AI-driven, reproducible measurements.

Seamlessly integrates into standard carotid protocols alongside Color and Pulsed-Wave (PW) Doppler.

Enhances preventive cardiovascular screening and vascular age assessment.

Hello. Carotid IMT or intima media thickness, is a measurement of the inner two layers of the carotid artery wall, the intima, and the media. The measurement is an indicator of subclinical atherosclerosis, and therefore can be used as a screening tool for vascular age or to determine cardiovascular risk.

So the carotid IMT measurement and the AI can be used with the Clarius L7, the L 15, and the Pal scanner. And you'll want to ensure that you're using the carotid preset. So I'll show you how it works. So first of all you're going to locate the carotid artery. And typically this measurement is done about a centimeter below the bulb proximal to the bulb or the flow divider.

The other thing you want to make sure of is that you're nice and perpendicular to that posterior carotid artery wall, and that's going to ensure the best image quality. You're going to get a more accurate trace. I also like to zoom my image to ensure that I'm seeing the carotid intima media layer, which I am there.

So when I'm ready, I'm going to tap the tools menu and select IMT from the tools menu. This is going to freeze my image. Place a yellow ROI box and perform a trace between the calipers of the intima and the media thickness, and the number that's displayed is an average of all the points along that trace. You can also adjust the size of this color box.

And this just totally depends on your protocol.

When I capture my image, the image as well as the measurements will be stored to the report and the various cloud. So the carotid auto IMT is available for you as a quick and easy, reliable and reproducible way that you can perform intima media thickness screening or help in the determination of your patient's cardiovascular risk.

It can be used as an adjunct to the tools that are already available to you in the carotid preset, such as color and pulse, wave Doppler, and all of the measurement tools that are associated with that preset.
