i'm frenkel an emergency physician point of care ultrasound enables me to provide more effective analgesia to my patients by visualizing my target nerve and my needle in real time avoiding surrounding vascular structures and other anatomic structures where i don't want my medicine to go in this example i'll show how to identify the posterior tibial nerve under ultrasound guidance in preparation for a nerve block i'm using a linear scanner here on the nerve preset and i'm going to start on the medial aspect of the leg just proximal to the medial malleolus i'll identify the posterior tibial artery here and just next to it is the nerve and i can follow that structure proximal up the leg keeping the nerve in view to find the ideal location where i feel best about doing my nerve block where the artery has diverged and i have a safe trajectory where i can watch my needle come in in real time and then inject just around the nerve avoiding all the surrounding structures and provide effective analgesia to the sole of the foot for my patient
