ACL Block #1 – iPACK

The iPACK block (infiltration between popliteal artery and capsule of the knee) is used to selectively block the posterior compartment of the knee, maintaining sensorimotor function of the leg/foot.

okay we got a young man for an ACL today this is Dr Greg Hickman from the Andrews Institute having an ACL so we're going to start with an iack injection then do a superior lateral genicular injection then put an inductor Canal catheter in using our clarus um Wi-Fi ultrasound probe along with uh with our uh mini iPad as our screen to be able to see what we're doing with this Wireless ultrasound so we're going to start with the iack injector we're going to start posteriorly so this we need a lot of depth so all I got to do is swipe my finger up like this add some depth to our image and little gain and I'm got to put a little pressure on here so here we can see the the artery go down just a little distal we should see there's the uh conds and we w't be just above that and I'm actually going to de increase the depth Just a Touch here now I can see the posterior femur and I'm going to come in between the femur and the artery you see the artery pulsating in the middle of the screen so we're going to come in from the right side and work our way across and infiltrate between the Popa till artery and the posterior capsule of the knee thus the ey pack so a little numb and shot here's a big old thing now on three one two three there we go see the needle coming in that's really just our skin infiltration there and then here we go okay here's our needle coming across the screen get down a little closer to the Bone to the femur here actually I'm hitting it get over now try to go down oh man okay Rocky give me five right there going to infiltrate two okay we're over medial to the poil artery and I'm going to come back closer to under the popil artery give me five more right here see the needle really well pull back here there's five CC's and give us five more CC's here 15 okay there's 15 CC's that's what we're going to do right there so normally get a little bit try to get a little bit closer to the femur this time the my entury point sort of kept me from doing that but we got a nice infiltration between the posterior capsule of the knee there the posterior femur and the poblo artery that's beaten right in the middle of the screen the nerves the Tubi panal nerves are going to be superficial so above the artery should be way out of the way of our infiltration down near the near the posterior capsule of the knee okay
