Dr. David Rosenblum, Director of Pain Management at AABPP Integrative Pain Care in Brooklyn, New York, has spent over a decade refining his practice using ultrasound guidance.

Choosing the Right Tool for the Setting: Handheld vs. Cart-Based
While Dr. Rosenblum typically uses a cart-based system at the hospital, the office setting allows him to take advantage of the portability of handheld units. He finds that the handheld device offers a specialized solution for a busy, multi-location practice.
In the office setting, I use both. I do use the handheld quite a bit. I find it easy when I’m going from my office to the hematology surgical center downstairs, and when I need to travel… it’s lightweight and it fits in my bag”, he explains.
Because the image quality is comparable to larger, more expensive machines, Dr. Rosenblum now rarely relies on his cart-based systems for office procedures.
Enhancing Safety and Diagnostic Accuracy with Ultrasound Guidance
Whether using a cart or a handheld, Dr. Rosenblum emphasizes that ultrasound provides a level of visualization that X-rays simply cannot match. By allowing clinicians to see “the pathologic structure,” ultrasound helps distinguish between nerves, blood vessels, tendons, and ligaments in real-time.
The ultrasound creates a safer situation in which I can avoid blood vessels or vital organs, so it definitely improves the safety of my procedures”.
This diagnostic clarity often leads to better outcomes. During a recent shoulder injection, Dr. Rosenblum used ultrasound to identify unexpected fluid around the biceps tendon, allowing him to treat the additional pathology during the same visit.

According to Dr. Rosenblum, patients have come to expect a high degree of precision for costly regenerative medicine like PRP or bone marrow aspiration and appreciate his use of ultrasound guidance during their procedures.
They like seeing for themselves the actual needle going in and the injection… When you’re doing the ultrasounds correctly, you can see the screen [with] a higher degree of satisfaction”, he says.
